Leave summary out and Heimdall drafts the opening prose from the record, which spends the team's AI budget; send it to keep Heimdall out. Answers 201 with the document.

A document to generate or change. On a create only kind is required; the title defaults to the kind's name and the format to PDF. On a change a field left out keeps its value; the kind never changes. Every change renders the document again on the queue.

An id in the path names nothing the token can see. TENANT_MISMATCH: the project sits outside the token's scope:project: allow-list, or the token carries no team scope. Both answer 404 rather than 403 so that existence outside the token's scope cannot be inferred.

The payload broke a rule, and error.fields maps each offending key to its messages. A refusal from the domain, such as a role code already taken or a team the caller does not own, uses the same code with error.message saying why and no fields.
On this endpoint: VALIDATION_FAILED: when the kind or the format is not open yet, the title is outside 3 to 160 characters, or the baseline named is not a frozen baseline of the project.

The first request with this key is still running; retry in a few seconds and the original response is replayed.
